PLE Examinee Guide: PRC Annexes A, B, and C

Good day Future MDs, welcome to PPC's blogs 

This blog will focus more on the Annex A, B, and C, on my original blog or guide (E. During the Exam portion), I had written there about the annexes, however, due to some changes that happened esp March 2023 batch here in NCR that there is QR code and now it the type of Annex Forms that you will submit differs depending on the exam site where you will take the exam, I decided to write this as a supplementary blog in which can guide you where you will take the exam. The Goal of this blog is to help the examinees or guide them wherever you will take your Physician Licensure Exam and what form you will pass

1. I wrote this on my own will voluntarily, to help you out with your application. Take note that everything I have written here is BASED ON MY EXPERIENCE LAST NOVEMBER 2021 and EXPERIENCES AND FEEDBACK OF MY FRIENDS UP TO MARCH 2023  which includes the Annexes for Annex A, and B which is a requirement of PRC to be passed on the 1ST DAY OF THE EXAMINATION, and Annex C which is a requirement to be submitted 15 days after the last day of the exam.

A. INTRODUCTION TO ANNEX A, B, AND C 

Nowadays, the PRC due to Memo of 2020-68 which requires the board examinees to pass Annexes A to C which are the following in relation to the COVID-19 pandemic [pls note I suggest you read the memo for more information which includes the discussion on COVID-19 protocol during the exam

Annex A- Informed Consent
Annex B- Health Declaration Checklist
Annex C- Post Examination Health Surveillance form 

Annex A and Annex B are part of the requirements of PRC which is usually submitted on the 1ST DAY OF YOUR EXAMINATION, as per experience and as per feedbacks of later batches, this is done usually in the time of submission of other requirements such as vaccination card etc............ (as listed when your respective schedule is posted on PRC)

2. On the Site please look for PHYSICIANS (take note there are 2 PHYSICIANS on the schedule (usually in MARCH AND OCTOBER)

3. Click on this and look for the 1st day schedule and you will find out which among the following will be the requirements to submit on the 1st day

***PS: Some as per feedback for the vaccination card prefers Printed VaxCert [which by experience is free and done online]; for the link of the VaxCert pls click on the url link:  https://vaxcert.doh.gov.ph/#/
  
As you can see in the picture above there is Health Declaration (Annex B) and Informed Consent (Annex A)

B. HOW TO KNOW IF YOU WILL SUBMIT MANUAL OR AUTOMATIC (QR CODE) ANNEXES
- For this it is usually indicated on the VERY FIRST PAGE OF YOUR ROOM ASSIGNMENT or BELOW NOTICE IN YOUR ROOM ASSIGNMENTS in the PLE which is released as per experience from 5 days to 2 weeks before the exam in this website: https://www.prc.gov.ph/room-assignment

As per feedbacks it is usually submitted in Manual format, but if there is a QR code of annexes like the one from NCR above, then it will be an automated QR code which you will fill out via google docs  

**To be safe please check if you will use Manual or QR Code since it will DEPEND ON YOUR EXAM SITE on what you will submit

C. MANUAL  ANNEX A, B, and C 

Manual Annex A, B is the one that in you will print a form of Annex A and B for 1st day then you will submit it
and Manual Annex C will be passed on the e-mail provided to you by paper on the last day of the exam at 15 days after the exam

There are two ways which you can download these Annexes

Okay let us bisect on the information which needed to be filled up
**Take note for Annex A and B as per experience, this is done on handwritten (I just used Adobe Acrobat here for the sake of demonstration purposes. 

MANUAL ANNEX A



For the Relative, as per experience your parents can be your witness and signature, date signed as per experience I did this at 1 day prior to exam

MANUAL ANNEX B


For the email address, to be safe, as per experience I used the email I use in PRC LERIS





Fill up the blanks Check and check tick what applies to you 

Fill up the following as instructed (as per experience the date I used here is 1 day prior to exam)

MANUAL ANNEX C
This is to be passed on 15 days after your exam via email on the e-mail address provided to you by the room watcher or proctor on the last day of your exam, PLS DO NOT LOSE THAT PAPER since it is important and where you will submit or attach your Annex C

**In my experience I passed this as a PDF file format, in my case, in Adobe Acrobat, there is a feature called "FILL AND SIGN" which you can sign the information properly, if you downloaded it via MS Word, you can convert it into pdf via Save As and Change the "Microsoft Word Document (.docx)" to "Portable Document Format (.pdf) then submit at email provided

But please take note, always check for any announcement in the PRC Regional Office on your exam site on how to submit this forms alongside with the other requirements especially during the 1st day

Example of submission of Annex C e-mail (take note look at the paper given to you at the end of your exam, if none, ask your proctor to be safe)

D. AUTOMATIC OR QR CODE ANNEX A, B, AND C

**This part is based from Feedbacks  

 This is the Forms of Annex A, B, and C which is passed by filling up a certain google docs that will appear on the phone or any gadget that you will use to scan the QR CODE,

Take note in the 1st Page of your room assignment you will see this kind of page

In the 1st page there is a separate QR Code for Annex A and Annex B when you scan it, these are the page that can appear 
Important things, 
1. Make sure that you are logged in using the email that you had used in PRC LERIS to be safe
2. Pls fill up the following information on the page, and take note of the following.
Licensure Exam: Choose Medicine
Date of Licensure Exam 
 From 1st day of Exam
 To: Last Date of Exam

Upload Valid ID (preferrably valid government ID) 


For Annex B, you will fill this out, follow the instructions that I had written on the manual except this time it is electronic and you will use phone, tick what is applicable to you

**For these 2 Annexes, as per feedbacks esp from March 2023 batch, it will depend on the proctor or testing site on how you will present the Annex A and B done screenshot. Some requires you to print it, in some, screenshot will do, in others they don't look at, it really depends on the testing site where you will take your PLE.

QR CODE ANNEX C
- For the QR Code of Annex C, it is usually found on your testing site it is printed on a piece of paper like this

Once you had seen this please take a picture of the printed QR Code, since you need to pass the Annex C 15 days after the last day of the exam. I suggest for the examinees with QR Code testing sites, do not leave the testing site especially at the last day without taking a picture of the QR code to avoid forgetting to fill the Annex C

Assuming it is 15 days (or depends as instructed) and now you will submit the Annex C, please scan the QR code and this screen should appear.
Fill up the following information provided, let me emphasize on this part on the form 
On the picture above, just like the manual form of Annex C, refer to your room assignment to fill up the following information above.
